๐จ 60-Second Triage โ A knock or heavy rattle that rises with revs, on any 2016-2020 Pilot: stop driving it and get it looked at before the next trip. That is the pattern behind NHTSA investigation PE25-008. Engine stall or power loss on a 2023-2025 Pilot: recall 25V-031, a free reprogram. Clunk or looseness from the rear suspension on a 2016-2022 Pilot in a salt state: recall 26V-365, subframe corrosion โ do not tow or load it until the VIN is checked. Engine will not restart at a stop light on a 2016-2021 Pilot: item 1, free under a settled class action. P0420 on a 2016-2019 Pilot: do not buy a converter yet โ item 5.
A Honda Pilot does not break at random. It breaks on a schedule, and here that schedule is unusually well documented โ Honda wrote most of it down, and a federal investigator wrote the rest.
One sentence should decide how you read the rest. Honda's connecting-rod bearing recall, NHTSA 23V-751, lists the Pilot as 2016, 2018-2019. Not 2016 through 2019: the 2017 is not in it, and neither is 2020. In August 2025 NHTSA opened PE25-008 into the same failure, listing 2016-2020 Honda Pilot and an estimated 1,410,806 vehicles, on the explicit finding that these failures fall outside the recall. Own a 2017 or a 2020 and you are in the investigation and not the recall. Six of the ten problems below carry a recall, a warranty extension or a settlement. Four still pay today; two have closed, and this guide gives the dates. By the end you will know which of the ten is due at your mileage, what each costs in 2026, and the cheap cause that gets skipped when the expensive one is quoted first. How to use this guide
Find your model year first, then your mileage. Every entry has the same parts: the tell, what it actually is, the 2026 price, the cheap cause that gets skipped, and where this does not apply. Three include a do-it-yourself path with honest limits, and every one ends with the sentence to say at the counter.
One rule overrides everything else: on a Pilot, the model year in a Honda document is the whole document. Not the generation, not "2016 and up." NHTSA's letter back to Honda asks the company to explain the gaps in model years. An adviser who says "that recall covers your year" without running the VIN is guessing, and the guess is worth five figures. Every year range below is copied from its document, never rounded โ and where two documents disagree, that disagreement is the point.
First: which Pilot do you actually have?
2003-2008, first generation. The transmission generation. CarComplaints ranks 2003 the worst Pilot year at 257 complaints, transmission failure averaging $3,470 at 123,600 miles. No current recall or verifiable nationwide extension covers it.
2009-2015, second generation with VCM. The misfire generation. Two Honda warranty extensions covered fouled plugs and piston-ring wear, and both have run out. Item 6 has the dates.
2016-2022, third generation. The heaviest document load: rod bearings, the nine-speed, Auto Idle Stop, fuel injectors and subframe corrosion, most with a campaign number.
2016-2021 with the nine-speed and Auto Idle Stop. A settled class action plus a ten-year extension on the no-restart fault. Item 1.
2023 and newer, fourth generation. Too new for a settled pattern, not too new for recalls: 25V-031 covers a software error that can stall 2023-2025 cars, and 26V-424 covers subframe bolts on 2026 cars. Treat published failure mileages here with caution, ours included.
The 10-minute check that dates your Pilot better than the odometer
- Run the VIN through NHTSA's recall lookup and Honda's owner site. Six of the ten items below can be answered by it.
- Compare your model year against two lists: 2016, 2018-2019 for the recall, 2016-2020 for the investigation. In the second and not the first, and item 4 is yours.
- Cold start it and listen to the first fifteen seconds, bonnet up. A metallic knock that rises with revs is item 4, not a wait-and-see noise.
- At a warm stop, watch whether Auto Idle Stop restarts cleanly. Any hesitation or no-restart is item 1, and it is free.
- Get codes read free and write every one down, including pending. P0301 through P0306, P0420, P0430, P219A and P219B each point somewhere.
- Look where the rear suspension bolts to the subframe. Flaking or perforated metal on a 2016-2022 salt-state car is item 8, today.
- Find the timing belt in the service record. No entry past 90,000 miles means item 9 is due, on an interference engine.

What actually determines your number
Which single model year is on your title. A 2016 is in the rod-bearing recall; a 2017 is not, but is in the investigation. A 2015 is in the VCM extension; a 2016 is not. One year separates a free engine from a five-figure quote.
Whether a campaign covers it. Recall remedies are free, and extension work is free within its terms. Six items here carry a document โ the largest cost variable on the vehicle.
How early a knock was investigated. Rod bearing failure is progressive, and the window between an audible noise and an unrebuildable engine is weeks of driving.
Whether the roads where it lived get salted. The subframe recall covers only vehicles sold in 22 states and DC. On a dry-state Pilot item 8 barely appears; on a Michigan car it outranks half this list.
Independent versus dealer. Typically 30-50% on labour-heavy work, but campaign work can only be done by a Honda dealer. The full comparison maps which jobs go where.
Whether the control software is current. The nine-speed's shift quality and the 2023-2025 stall recall both come down to a reprogram. Software before parts is the actual repair in two of these ten.
Whether you bought it used with no history. Assume no campaign is complete until the VIN says so, and the belt undone until an invoice proves otherwise.
The ten problems, in the order they arrive
1 ยท Auto Idle Stop will not restart the engine โ 20,000 to 80,000 miles
The tell. You stop at a light, the engine shuts off as designed, you lift off the brake โ and nothing happens. Sometimes it cranks and catches late; sometimes not at all.
What it actually is. The class complaint puts it plainly: the engine may not automatically restart when a driver releases the brake pedal, leaving the vehicle temporarily undrivable. The hardware end is the starter motor assembly, and Honda has been reimbursing starter replacement, starter relay replacement and valve adjustment.
The cheap cause that gets skipped โ and it is free. Before anyone quotes a starter, ask for the campaign. Under the settlement Honda amended Honda Service Bulletin 23-009 โ covering 2019-21 Passport, 2016-2021 Pilot and 2020-2021 Ridgeline โ specifically to eliminate AIS symptom verification as a condition to receiving the repair. If the vehicle has had the software update, the dealership cannot require a technician to reproduce the no-restart before replacing the starter free. Being unable to reproduce an intermittent fault was how owners got sent home.
Where this does not apply. A narrow list, and widening it gets you turned away: 2016-2021 Pilot only, with a NP0 engine, the nine-speed and Auto Idle Stop fitted. A 2015 is outside it, a 2022 is outside it, and a six-speed car never had the feature. A vehicle already given an A53 starter free under warranty is excluded except for reimbursement. The deadline for claiming money already spent closed August 18, 2026.
At the shop โ say this: "This is a no-restart on Auto Idle Stop. Please check the VIN against Honda Service Bulletin 23-009 and the Auto Idle Stop warranty extension. Under In re Honda Idle Stop Litigation the symptom verification requirement was removed, so I shouldn't have to reproduce it for you. I'd like the coverage documented on the invoice."
2 ยท The nine-speed jerks, hunts or hesitates โ 20,000 to 90,000 miles
The tell. A shove between gears at low speed, hunting on a gentle hill, a beat's delay before it picks a gear. Worst cold, worst in traffic.
What it actually is. The control module learns your driving to pick among nine ratios, and when those adaptation values drift, shift quality goes with them. The everyday fix is a module update and a fluid service, which is why CarComplaints logs 34 transmission-jerk complaints on the 2016 Pilot averaging $290 at 53,600 miles โ a software-and-fluid number, not a rebuild.
The cheap cause that gets skipped. Ask for the current control module calibration and a fluid condition check first โ a Pilot transmission fluid change is $166 to $208. Our transmission repair guide covers the escalation order; transmission slipping covers the serious symptom.
Where this does not apply. No NHTSA recall and no Honda campaign covers shift quality itself on the nine-speed โ if somebody says otherwise, ask for the number. And this is not item 1, a settled class action about failing to restart at a stop. 2016-2018 Pilot LX and EX two-wheel-drive cars had a six-speed, so none of this applies to them.
At the shop โ say this: "Before we discuss anything internal: is the transmission control module on the latest calibration for this VIN, and what's the fluid condition? I want both on the estimate, with the adaptation values reset and a road test, before any teardown is quoted."
3 ยท Front brakes on a heavy three-row โ 25,000 to 60,000 miles
The tell. A thin high squeal at low speed that goes away under hard pressure โ the wear indicator doing its job. Grinding is low and gritty, and by then you are buying rotors too.
What it actually is. Ordinary consumption on a two-tonne three-row SUV. RepairPal puts Pilot brake pad replacement at $313 to $367 โ $235 to $292 on a 2024, $353 to $410 on a 2012. Where this does not apply: nothing here is Pilot-specific, and a pedal that pulses rather than squeals is rotor runout, not pad wear.
The cheap cause that gets skipped. A squeal on the first two stops of a damp morning is surface rust and costs nothing. Ask for pad thickness in millimetres; above 4mm you are buying time you already own. Our braking-noise guide separates the two sounds.
At the shop โ say this: "What's the actual pad thickness in millimetres, front and rear, and are the rotors above minimum spec? If the pads are over 4 millimetres I'd rather wait and re-measure at the next service."

4 ยท Connecting rod bearing wear and engine failure โ 30,000 to 120,000 miles
The tell. A metallic knock or deep rattle from low in the engine, rising and falling with engine speed rather than road speed. Often loudest on a cold start and quieter once oil pressure comes up, which is why it gets postponed. Some owners get an oil-pressure warning first; some get nothing until the engine stops.
What it actually is. Honda's recall language is worth reading twice: due to a manufacturing error the connecting rod bearing may wear and seize, damaging the engine, and a damaged engine may run improperly or stall while driving, increasing the risk of a fire, crash, or injury. NHTSA 23V-751 covered 248,999 vehicles, its Pilot line reads exactly "2016, 2018-2019", and dealers inspect and repair, or replace the engine, free of charge.
The part almost nobody explains. On 20 August 2025 NHTSA opened PE25-008, subject "Engine failure", into rod bearing failures in the 3.5-litre V6 covering 2016-2020 Honda Pilot among other models. Estimated population 1,410,806; 3,012 reports, 7 involving crashes or fires. It exists because NHTSA determined these failures fall outside 23V-751. Own a 2017 or a 2020 and that is your document.
The cheap cause that gets skipped. Not every knock is a bearing: a loose heat shield, an exhaust flex joint or a worn tensioner pulley are free to identify with a stethoscope on a lift. Insist that check is shown to you before a teardown is quoted โ and do not let a shop stop at "it's just a heat shield" on a 2016-2020 car. What this costs outside a campaign. One reported case: a 2017 Pilot at 56,000 miles, diagnosed with rod bearing failure, quoted nearly $25,000 by the dealership for a complete engine, Honda declining as out of warranty. One quote on one vehicle, not a range and not an average โ included because it shows what the gap between the two lists is worth.
Where this does not apply. The recall does not cover a 2017 or a 2020; the investigation does not cover a 2015 or a 2021; neither covers the first, second or fourth generation. An open investigation is not a recall โ it creates no entitlement to a free repair.
At the shop โ say this: "There's a knock that changes with engine speed. Before anything else, please check this VIN against recall 23V-751. If my year isn't in it, I know NHTSA investigation PE25-008 covers 2016-2020 Pilot for connecting rod bearings, and I want the diagnosis documented so I have a record either way."
5 ยท Fuel injector wear that gets billed as a catalytic converter โ 40,000 to 120,000 miles
The tell. A check-engine light with P0420 or P0430, often alongside P0300 through P0306, P219A or P219B. Often no symptom at all, and the first sign is a failed emissions test or a converter quote.
What it actually is. Honda's memo is direct: the injectors may not have been manufactured to proper specifications, and debris from the high pressure pump or the machining process may cause internal wear or clogging. Then the sentence that saves people thousands: the PCM software may misinterpret sensor inputs as a deteriorated catalytic converter. The manufacturer stating, in its own document, that this fault presents as a bad converter and is not one.
The cheap cause that gets skipped โ free if you are in scope. Honda extended fuel injector coverage to 10 years from the original date of purchase or 150,000 miles, whichever comes first. The November 24, 2020 memo covered certain 2016 Pilot vehicles under Service Bulletin 20-100; the February 11, 2021 expansion added certain 2017-19 Pilot vehicles under 21-010. Together, Pilot coverage runs 2016 through 2019. Against a converter estimate of $2,621 to $2,815, one VIN status inquiry is the best-paid minute available here.
The confirmation test. Ask whether injector codes or fuel-trim imbalance are stored alongside the P0420; a genuinely dead converter rarely arrives with per-cylinder misfire codes.
Where this does not apply. The extension covers nothing before 2016 and nothing from 2020 on. Ten years from purchase is a real clock: on a 2016 it has run out, on a 2019 it has not. CarComplaints lists fuel injector malfunction as the worst-reported problem on the 2016 Pilot at an average of $1,300 and 59,000 miles โ roughly what it costs once the clock has run out.
At the shop โ say this: "Before you quote a catalytic converter: please do an iN VIN status inquiry against Honda Service Bulletins 20-100 and 21-010, the fuel injector warranty extension. Honda's own memo says the software can misinterpret this as a deteriorated converter. I want that written on the invoice before anything is ordered."
Why does one engine noise on a Honda Pilot range from nothing to five figures? Because the same knock has a free outcome and a catastrophic one, and the thing that separates them is the model year on your title rather than the sound itself. Honda's recall 23V-751 lists the Pilot as 2016, 2018-2019, and inside that list dealers inspect and repair, or replace the engine as necessary, free of charge. NHTSA's investigation PE25-008, opened on 20 August 2025, lists 2016-2020 Honda Pilot and an estimated 1,410,806 vehicles, on the finding that those failures fall outside the recall's scope. A 2017 owner therefore has the defect described in a federal document and no free remedy attached to it, and one such owner was quoted nearly $25,000 for an engine at 56,000 miles. Meanwhile a loose heat shield makes a similar noise for nothing. Pulscar's free sound check reads that cold start, which is when the difference is audible.
6 ยท VCM misfire, fouled plugs and piston ring wear โ 60,000 to 150,000 miles
The tell. A rough idle or stumble that comes and goes, codes in the P0301 to P0304 range, and oil consumption creeping up with no drip on the driveway.
What it actually is. Both Honda memos describe the same chain. The 2025 one: piston ring effectiveness may be reduced due to excessive wear and oil sludge build-up, which can cause oil to adhere to the spark plug, potentially leading to carbon fouling and misfire triggering DTCs P0301 through P0304. Oil gets past the rings, the plug fouls, the cylinder misfires.
The cheap cause that gets skipped. Plugs and coils, in that order, before anyone says the word rings. RepairPal puts Pilot spark plug replacement at $305 to $450, and $179 to $362 on a 2011; an ignition coil is $132 to $158. The free test comes first: if the code names one cylinder, swap that coil with a neighbour. If the misfire follows the coil it is the coil; if it stays put, the problem is in that cylinder.
Fix it yourself โ the front bank only. The three front plugs on a transverse V6 are reachable with a plug socket and an hour. The honest limit: the rear bank sits against the bulkhead, and reaching it means pulling the intake manifold, with new gaskets and a real chance of dropping something into an open port. Hand those three over.
Where this does not apply, and the dates that matter. Neither extension is live. The October 2013 memo covered "2008-12 Accord V6, 2010-12 Crosstour V6, 2008-13 Odyssey, and 2009-13 Pilot" for 8 years, unlimited mileage โ long expired. The June 2025 memo covered certain 2013-2015 Pilot and 2013-2017 Odyssey vehicles under bulletins 25-060 (Pilot) and 25-061 (Odyssey), on terms of 10 years from the original date of purchase or 150,000 miles, whichever comes first, with cover regardless of time or mileage only until January 31, 2026. That window is closed and the ten-year term has run out for nearly everyone. Run the VIN anyway โ a late-sold 2015 may still be inside it. None of this touches a 2016 or later Pilot.
At the shop โ say this: "Misfire codes in the P0301 to P0304 range on a VCM engine. Please do an iN VIN status inquiry against Service Bulletin 25-060 first, then a coil swap test on the named cylinder, then plugs. I want a compression or leak-down result before anyone quotes internal work."
7 ยท VTM-4 rear differential judder โ 60,000 to 150,000 miles
The tell. A judder or groan from the back in tight, slow turns โ a car park manoeuvre, a three-point turn. Felt more than heard, absent at speed.
What it actually is. The rear differential runs clutch packs to distribute torque, and those clutches share their fluid with everything they wear into it. Old fluid stops damping the engagement, and the clutches grab and release in a way you feel through the floor โ one of the few complaints where the diagnosis and the repair are the same action.
The cheap cause that gets skipped. The fluid is the repair. RepairPal puts a Pilot drive axle assembly drain and refill at $107 to $147, of which the fluid is about $22. A shop opening with a differential rebuild has skipped step one: ask for the fluid changed and a road test on the same turns first.
Fix it yourself โ genuinely realistic. A fill plug, a drain plug, a pump bottle and twenty minutes on level ground. The honest limit: use the fluid Honda specifies and nothing else, because the wrong friction modifier makes the judder worse โ and crack the fill plug before the drain plug. Drain first and find the fill plug seized and you own a vehicle you cannot refill.
Where this does not apply. Two-wheel-drive Pilots have no VTM-4 unit, so this item does not exist for them. A judder felt through the steering wheel, or one at speed, points at suspension instead โ our guide to noises when turning covers those.
At the shop โ say this: "Judder from the rear in tight slow turns on an all-wheel-drive Pilot. Please change the VTM-4 rear differential fluid with the Honda-specified fluid and road test it on the same manoeuvre before quoting anything else. I want the fluid condition noted on the invoice."
Which Honda Pilot year is the one to be careful about, and why is it the one nobody names? The 2017. It sits in the middle of a recall list that skips it: NHTSA 23V-751 covers the Pilot as 2016, 2018-2019, so a 2017 owner with the identical connecting-rod bearing failure has no free remedy attached to their VIN. The same year is squarely inside NHTSA investigation PE25-008, opened in August 2025, which lists 2016-2020 Honda Pilot and an estimated 1,410,806 vehicles across five models. The 2020 sits in the same position. Neither year is worse-built than its neighbours; they are simply outside a campaign, which is a paperwork status rather than a mechanical one, and paperwork status is what determines who pays. A used listing will never mention it, and a service adviser reading a recall screen will say you are clear. Pulscar's free sound check reads the cold start where a rod bearing first becomes audible.
8 ยท Rear subframe corrosion โ 80,000 miles and up, salt states only
The tell. Often nothing until an inspection. When there is a symptom it is a clunk from the rear over bumps, or a rear end that feels vague. Underneath you may see flaking or perforated metal where the rear suspension arms bolt to the subframe.
What it actually is. Honda's filing is short: the rear subframe may corrode at the suspension mounting points, resulting in rear suspension component failure โ filed as a loss of vehicle handling and control. NHTSA 26V-365 was acknowledged June 09, 2026, covers 880,514 vehicles, and its Pilot line reads 2016-2022. Honda's numbers are AOU and AOT.
The cheap cause that gets skipped โ here it is the whole repair. The remedy is free: dealers inspect the subframe and install a reinforcement kit and, as necessary, repair or replace components. If an independent shop has quoted subframe work on a Pilot in this range, stop and run the VIN.
Where this does not apply. The recall covers vehicles sold in Connecticut, Delaware, District of Columbia, Illinois, Indiana, Iowa, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, Virginia, West Virginia or Wisconsin. A Pilot sold and driven its life in Arizona is outside it. It does not cover 2023 and later Pilots. And it is not NHTSA 26V-424, a separate, far smaller recall covering 2026 Pilot and Passport vehicles whose rear subframe bolts may have been tightened improperly โ different years, different cause, Honda's number OOY.
At the shop โ say this: "Please check this VIN against recall 26V-365 for rear subframe corrosion. If it applies I want the reinforcement kit installed and the inspection documented, and I'd like photographs of the mounting points either way."
9 ยท Timing belt and water pump โ 90,000 to 110,000 miles
The tell. Usually none, which is the problem. Sometimes a light rhythmic ticking from the front of the engine, sometimes a coolant seep behind the timing cover. Most owners find out because somebody read the service record.
What it actually is. A rubber toothed belt drives the camshafts โ a wear item with a published interval, not a lifetime part. RepairPal puts Pilot timing belt replacement at $1,117 to $1,547. The water pump runs off the same belt: alone it is $863 to $1,271, of which $506 to $742 is labour a belt job already pays for.
The cheap cause that gets skipped. The cheapest decision on this page: have the water pump, tensioner and idlers done in the same job as the belt, because doing the pump separately pays that access labour twice. The document that applies to a narrow slice of owners. NHTSA 19V-298 covers certain 2018-2019 Acura MDX and Honda Pilot vehicles plus several 2019 models, where the timing belt teeth may separate from the belt, filed as an engine stall risk. Dealers inspect and replace any affected belt free, and vehicles receiving one also have the engine inspected. Honda's number is L4M; 93,946 vehicles.
Where this does not apply. 19V-298 is a defect campaign on a specific build population โ not a free scheduled belt service, and it does not reset your maintenance interval. It does not cover 2016, 2017, or 2020 and later Pilots. And this is an interference engine: if the belt goes, valves and pistons occupy the same space.
At the shop โ say this: "I'd like the timing belt done with the water pump, tensioner and idlers in the same job, quoted as one bundled labour operation. Separately, please check the VIN against recall 19V-298 in case this car is in that population."
10 ยท Catalytic converter and the P0420 that often is not one โ 120,000 miles and up
The tell. A steady check-engine light with P0420 or P0430, usually with no drivability complaint. Frequently the first sign is failing an emissions inspection.
What it actually is. The computer compares the oxygen sensors before and after the converter, and when their readings converge it sets the code. That says a monitor failed its test, not that the converter is dead. RepairPal puts a 2019 Pilot converter at $2,621 to $2,815, of which $2,231 to $2,242 is the part; on a 2005 it is $1,520 to $1,599.
The cheap cause that gets skipped, and here there are three. First: on a 2016-2019 Pilot this code may be a fuel injector Honda has agreed to pay for under Service Bulletins 20-100 and 21-010, because its own memo says the software may misread injector wear as a deteriorated converter. That is item 5, and it comes first here too. Second, a lazy downstream oxygen sensor sets the same code for a fraction of the money. Third, an unresolved misfire poisons the substrate, so on a 2013-2015 car the real repair is upstream in item 6.
The confirmation test: Mode 6. Mode $06 reports the catalyst monitor's actual result as a number against its threshold rather than a pass or fail. Ask for those values in writing first. Our guide to expensive codes covers the pattern; the converter cost guide covers a fair price.
Where this does not apply. If a misfire code is stored alongside P0420, resolve the misfire first โ fitting a converter while the thing that poisoned it still runs is money spent twice. And on a 2020 or later Pilot the injector extension does not apply, so the cheap route is the sensor and the Mode 6 numbers rather than a VIN inquiry.
At the shop โ say this: "Before we replace the converter I want three things: the Mode 6 catalyst values against threshold, the downstream sensor actually tested rather than assumed, and an iN VIN status inquiry against Service Bulletins 20-100 and 21-010."
The price ladder: read it from the bottom up
Read it from the bottom up. The top lines are free, and between them they address the two at the bottom. What decides which end you land on is a model year and a phone call.
Your number, by which Pilot you drive
Two rows invert this guide rather than refining it. The 2017 and 2020 row reverses the usual advice: on most vehicles a year outside a recall is good news, and here it is the bad news, because the failure is documented federally while the free repair is not attached to your VIN. And the salt state row reorders everything, because on a Michigan Pilot the corroding subframe is the only item filed as a loss of vehicle handling and control.
Should you buy a used Honda Pilot? The best and worst years, and how to check in five minutes
Most people searching for Pilot problems are deciding whether to buy one. The Pilot is an ordinary three-row SUV: RepairPal rates it 3.5 out of 5.0, ranks it 13th of 26 midsize SUVs, and puts average annual repair cost at $542, with a 13% chance any repair is severe against a 13% segment average. Unremarkable numbers, and not the reason to be careful.
What actually carries risk. One narrow thing: the rod-bearing question on the third generation. Everything else is free under a campaign, cheap, or ordinary wear.
The years with the strongest paperwork. A 2016, 2018 or 2019 is inside recall 23V-751, so if the engine has the defect Honda pays to repair or replace it. Counter-intuitively that makes those years easier to buy than the ones beside them.
The years to look at hardest. A 2017 or a 2020 โ not because they are built worse, but because they sit outside the recall and inside PE25-008.
The year with the deepest complaint history. 2003, at 257 complaints, transmission failure averaging $3,470 at 123,600 miles, no campaign behind it.
The five-minute pre-purchase procedure:
- Run the VIN through NHTSA's recall lookup. Open campaigns are free repairs you schedule after purchase, not deal-breakers.
- Ask about 23V-751, 26V-365 and, on 2023 and newer cars, 25V-031 โ and whether each was completed.
- Cold start it yourself, bonnet up, before the seller warms it. Fifteen seconds answers the question that matters on the third generation.
- Ask where it was sold. If it is one of the 22 states in 26V-365, get underneath it.
- Find the timing belt in the record past 90,000 miles, or treat it as a $1,117 to $1,547 conversation.
The short version: a Pilot with completed campaigns, a clean cold start and a timing belt invoice is a normal used SUV. A 2017 with an unexplained knock is a different vehicle, and the two look identical in a listing.
What actually happens at the shop, checkpoint by checkpoint
Checkpoint 1 โ the VIN question. "What open and completed campaigns does this VIN show?" First sentence, and free.
Checkpoint 2 โ the model year question. "Is my year inside 23V-751, or only PE25-008?" A screen showing nothing open does not mean the failure does not apply.
Checkpoint 3 โ the write-up. "Can you read back the complaint as written?" "Metallic knock, cold start, rises with revs" gets a different diagnosis from "engine noise."
Checkpoint 4 โ the diagnostic fee. "Does it credit toward the repair?" Roughly $80-$150 independent, $120-$200 dealer. What a diagnostic should cost has the detail.
Checkpoint 5 โ the software question. "Is the control module on current calibration?" On item 2 and the 2023-2025 stall recall, software is the repair.

Three diagnostic traps that cost Pilot owners the most
Trap 1: the recall screen that says you are clear. The situation: a knock from a 2017 Pilot at 74,000 miles. What the shop says: "No open recalls on this VIN. It's out of warranty." What is really happening: the screen is telling the truth and telling you nothing. 23V-751 lists the Pilot as 2016, 2018-2019, so a 2017 shows clear, while PE25-008 lists 2016-2020. Price versus bill: $0 to document it, against one reported quote of nearly $25,000. Your protective question: "My year isn't in 23V-751, but is it in PE25-008? Please note the diagnosis in writing."
Trap 2: the converter quoted on a car Honda still owes injectors for. The situation: P0420 on a 2018 Pilot at 96,000 miles. What the shop says: "Catalytic converter. Around $2,700 fitted." What is really happening: Honda's memo says the PCM software may misinterpret sensor inputs as a deteriorated catalytic converter, and Service Bulletins 20-100 and 21-010 cover injectors on 2016-2019 Pilots. Price versus bill: $0 inside the extension, against $2,621 to $2,815. Your protective question: "Can you do an iN VIN status inquiry against 20-100 and 21-010 before ordering a converter?"
Trap 3: the starter you are told to prove. The situation: intermittent no-restart at stop lights on a 2019 Pilot. What the shop says: "We couldn't duplicate it. Come back when it does it here." What is really happening: under the settlement Honda amended Honda Service Bulletin 23-009 to eliminate AIS symptom verification as a condition of the repair. Price versus bill: $0 under the ten-year extension, against $571 to $988. Your protective question: "Symptom verification was removed as a condition under the settlement. Can you check the bulletin and proceed on the VIN?"
Three quotes, decoded
These are composite scenarios, not individual customers โ the mileages, quotes and outcomes are typical of the pattern each item describes, assembled so the arithmetic is visible. We do not publish invented testimonials, so nothing below is presented as a specific person's story.
Scenario 1 โ the same noise, two model years apart. Two Pilots with an identical cold-start knock, a 2016 and a 2017, both near 70,000 miles. Run the VIN against 23V-751 and the 2016 gets an engine inspected, repaired or replaced free. The 2017 gets a screen saying no open campaigns and a paid diagnosis. Lesson: the invoice is decided by the model year before anyone opens the bonnet. Ask which list you are on, not whether anything is open.
Scenario 2 โ the $2,700 quote Honda may owe. A 2018 Pilot at 96,000 miles, steady P0420, no drivability complaint. Ask for Mode 6 values and a VIN inquiry against 20-100 and 21-010, and it may end with covered injectors instead of a converter at $2,621 to $2,815. Lesson: the manufacturer's own memo says this code can be a misread. One question costs nothing.
Scenario 3 โ the transmission that was software. A 2016 Pilot at 55,000 miles jerking between the lower gears. Ask for the module calibration and a fluid service before any teardown and you are in the territory of the $290 CarComplaints average at 53,600 miles. Lesson: a symptom that feels structural is not automatically expensive. Software and fluid come before the word rebuild.
How can you tell a real Honda Pilot failure from an unnecessary repair quote? Three checks cover most of it and all three are free. First, the campaign check: six items in this guide carry a document, remedies under a safety recall are performed at no charge, and completion is recorded against your VIN. Second, and specific to this vehicle, the two-list check: a screen showing no open recalls does not mean the defect does not apply, because 23V-751 covers the Pilot as 2016, 2018-2019 while PE25-008 covers 2016-2020. Third, the warranty-extension check before any catalytic converter, because Honda's own memo says the software can misread injector wear as a dead converter. Beyond those, ordinary discipline applies: a coil swap before deeper misfire work, fluid and calibration before a transmission teardown, Mode 6 values before a converter. "There's a knock and I have a 2016-2020 Pilot." This week. Check the VIN against 23V-751, ask about PE25-008 if your year is not in it, and get the diagnosis in writing.
"P0420 and someone quoted a converter." Do not authorise it. On a 2016-2019 Pilot ask for a VIN inquiry against 20-100 and 21-010, and Mode 6 values in writing. Both are free.
"It jerks between gears." Software and fluid first, adaptation values reset, road test. Only then does a paid internal conversation make sense.
"Nothing's wrong, I just bought it." Run the VIN lookup today and schedule anything open. Write your model year on a note with the two lists beside it.
After the repair: verify, then prevent
Verify within a week. Engine work: two full cold starts with no knock. Recall work: completion documented against the VIN in writing โ a verbal "yes we did that" is worth nothing later. Injector work: the light stays off through two drive cycles.
Record the before and after. Thirty seconds from the same position at cold start is the cheapest proof of repair here โ the recording guide has the protocol.
Prevent the next one. Oil on schedule and checked between services, because oil is what stands between a rod bearing and a bill. Plugs, coils and VTM-4 fluid on interval. And an annual VIN lookup: 26V-365 arrived in June 2026 for cars built a decade earlier.

Two things this guide could not verify, stated plainly. No NHTSA recall and no nationwide Honda warranty extension covering 2003-2007 Pilot automatic transmission failure could be found in a primary document, so that item is priced from owner-reported averages only. And no campaign covering nine-speed shift quality could be found in a primary copy โ a bulletin number circulates in owner forums, but without a document behind it this guide does not cite it.
